Igor proclaims that doctors are happiest when they can work, so he'd considered submitting Luka as a prize (the implication being that this is a present specifically for a cosmetic surgeon). He settles instead for submitting a roast suckling pig, saying that whenever a doctor took care of one of his loved ones, he would send such a gift to the doctor's house in thanks. Ivan says it's a good gift, but not necessarily doctor-specific, and awards third place to Igor.
